Chronic Care Management (CCM) is a Medicare program which provides beneficiaries with two or more chronic health conditions access to a care coordination program at little or no cost. You can enroll in CCM services through participating primary care physicians or directly through Servants Approach Consulting. 

We develop a customized ongoing care plan, coordinate with your doctor(s) and pharmacies, provide caregiver and patient education, update your electronic health record, manage medications,  and schedule medical appointments.

At Servants Approach Consulting, a board-certified pharmacist is assigned to you upon enrollment. Services begin within 7 days of enrollment. You have 24/7/365 telephonic or virtual access to a board-certified pharmacist to answer any care questions. Sign onto our web or mobile portal with the touch of a button to access your Personal Pharmacist.

Your Personal Pharmacist can also visit you at home or doctor’s office. With confidential appointments- based visits that run 20 to 60 minutes depending on the number of medications you are taking. During the visit, your Personal Pharmacist will inspect your medication profile and review all prescriptions, over-the-counter drugs (OTCs), herbal supplements, and samples for appropriateness of therapy. Servants Approach Consulting Pharmacists will discuss therapy- and medication-related issues or problems, and answer any questions you have about your medications and care plan.
Upon completion of the visit, you will receive a medication list and personalized care plans detailing the just-completed visit with the pharmacist. Plans include times and dates for follow-up visits. Your Personal Pharmacist will also update your doctor, coordinate further care, and relay medication or plan recommendations.
